We will keep your medical records accurate and up to date.  You have the responsibility to tell us any changes of your phone number, address or surname.  You have the right to see your computerised medical records and written medical records compiles after November 1991.  If you wish to do this please ask at reception for details.  A small charge is made for a copy.  All staff are aware of the need for confidentiality, and have received training in this.  Breach of confidentiality is recognised as grounds for disciplinary action. Rules of confidentiality mean your records will only be shard between health professionals involved with your care, and information is only divulged in reports with our permission, except where we are statutorily obliged to do so (for example court orders ore repots to check on disability claims).  Only you or your legal guardian will be given results of your test.  You can speak to a Receptionist in private if required.
