If you reside permanently within our Practice Area, your request to register with the Practice will, except in exceptional circumstances, be accepted.  For patients aged 18 or over you will  need to attend the surgery with 1) photo ID, 2) proof of your address eg utility bill, pay slip, and 3) proof of your date of birth eg birth certificate.  For under 18's we will require a copy of the birth certificate giving the mother's name.  You will be treated with respect and dignity whatever your cultural background or disabilities.  You can make an appointment with whichever doctor you choose. We prefer patients to follow a problem through with their chosen doctor to ensure, wherever possible, continuity of care.
